I received the following message from AFIA regarding a VFD form that had been available on theĀ American Veterinary Medical Association (AVMA) website. If you accept VFD’s :
“… the form was presenting problems. The note below from AVMA states that form was removed from the association’s website and has been replaced by some other AVMA resources for VFD:
“…the AVMA has changed its VFD resources for its members. Specifically, we have removed the fillable VFD form. AVMA members will no longer be able to fill out and print a VFD form from the AVMA’s website. We did update our Instructions for Completing a Veterinary Feed Directive (VFD) Order page to recommend members use one of the electronic services or to work with the pharmaceutical companies. However, veterinarians are not mandated to use one of these options and are allowed to use paper VFD forms. Therefore, we have kept the instructions on how to fill out a paper VFD form and reference the Common Format VFD form listed in Appendix A of the FDA’s GFI 233: Veterinary Feed Directive Common Format Questions and Answers.””
